The Intake & Triage Coordinator will be responsible for providing oversight and support for all incoming cases. Individual will also be responsible for receiving faxes, creating cases, distributing cases to team, and communicating with Case Managers, patients and healthcare professionals if information is missing (as appropriate within the Program Design).

Responsibilities

  • Review and ensure all incoming forms are complete and legible
  • Follow up with stakeholders as appropriate to collect and capture missing/incomplete/illegible information
  • Accurately capture program specific requirements from referral into Program Software Platform
  • Ensure timely processing of all forms/referrals based on pre-set program requirements
  • Create new patient profiles or accurately associate correspondence to existing patient profile
  • Assess request on forms and associate appropriate services/resources based on business rules and program guidelines
  • Identify and report pharmacovigilance information as required by client(s) (i.e., Adverse Events)
  • Answer and triage inbound calls to assigned case managers or appropriate partners (Program Specific)
  • Triage referrals and task assigned case managers regarding the appropriate next steps (Program Specific)
  • Work closely with internal team to appropriately manage and resolve issues (Program Specific)
  • Maintain confidentiality of patient and proprietary information (Program Specific)
  • On time adherence to training deadlines for all corporate policies and procedures governing access to confidential data
  • Ensure all SOPs are followed with consistency
  • Conducts miscellaneous tasks or projects as assigned

Qualifications

  • High school diploma or equivalent required. AA degree or some college preferred
  • At least one year of experience in a telephonic and form triage/intake/disease management environment
  • Demonstrate ability to communicate clearly in both written and oral communication
  • Proficient competency using Word, Excel and PowerPoint
  • Strong attention to detail
  • Strong interpersonal skills; team player
  • Adaptable and flexible to new situations
  • Ability to work assigned shift hours

Physical Demands & Work Environment

  • While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit for long periods of time, use hands to type, handle or feel; and reach with hands and arms. Prefer candidates who can type at least 35 words per minute with 97% accuracy.
  • Although very minimal, flexibility to travel as needed is preferred.
  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, etc.
